We are trying to understand problems that patients are having with requesting prescriptions through the NHS App and would be grateful if you could fill out the prescriptions questionnaire.

The easiest way to request repeat prescriptions is to download the NHS app for your phone from the Google Play or Apple Store. This allows you to easily request repeat medicines as well as access certain other services. See www.nhs.uk – The NHS App for more information.

If you do not have a Patient Access account, you can use this form to request any repeat prescriptions from the Practice.

Please allow 2 working days before collecting your prescription.

In future you may wish to consider registering for our Online Services. The Online Services system remembers which medications you are on and makes requesting repeat prescriptions faster and easier.
