NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.
In case of a life-threatening emergency, please dial 999.
To find out your recent test results requested by your GP, please contact the surgery 7-10 days after you had the test. You can contact us by submitting an online request by clicking here https://florey.accurx.com/p/P84010 or call reception on 0161 438 2821 after 2pm.
You can also view test results via the NHS App.
To get a sick/fit note, please submit an online request form by clicking here https://florey.accurx.com/p/P84010 or call reception on 0161 438 2821 if you need support with submitting a request.
For new sick/fit request you need to speak to a GP, please submit a medical query to arrange to speak with a GP.
For continuation of a sick/fit note, please use the Admin Query tab and choose “Fit (Sick) Note”.
Please include as much detail as possible including the dates and reasons.
