To find out your recent test results requested by your GP, please contact the surgery 7-10 days after you had the test. You can contact us by submitting an online request by clicking here https://florey.accurx.com/p/P84010 or call reception on 0161 438 2821 after 2pm.

For new sick/fit note requests you need to speak to a GP, please submit a medical query to arrange to speak with a GP.

For continuation of a sick/fit note, please use the Admin Query tab and choose “Fit (Sick) Note”.

Please include as much detail as possible including the dates and reasons.
